LCSD to Participate in Weight Status Survey

New York State school districts are selected annually to take part in a survey conducted by the New York State Department of Health. During the 2017-2018 school year, the Liverpool Central School District will be required to report aggregated student weight status data as part of this survey. This information will assist health officials in developing programs for students in New York State. No individual student names, BMI's or weight status group are included with the data.
You have the option of removing your child's weight status group information from the aggregated data sent to the New York State Department of Health.
If you do not wish to have your child's weight status group information included as part of the New York State Health Department's survey please download the opt out letter (by clicking here) and return it to the Health Office.
